Hello all,
Our office is rolling out a new platform for COI disclosures. Effective July 1, 2021 all COI disclosures will need to be made through the Cayuse Outside Interests platform. You can find more information on our COI webpage.
We have taken this opportunity to make the following changes to the form:
- EPAP (External Professional Activities for Pay) disclosures are now incorporated into the Annual COI disclosure form. You no longer have to complete a seperate form for your EPAP's.
- Questions have been updated for clarity
- Information saved on a prior disclosure form will transfer to the current disclosure form. If this information is still accurate, you can simply updated the information rather than having to start from scratch.
