IRB Staff have made some changes to the Exempt and Non-Exempt Consent Form Templates, as well as adding a Parental Permission Template, to improve the quality of applications. These templates can be found on the App State IRB website, in the Consent Guidance Templates tab.
Please be sure to read all directions (indicated in red) when editing the templates to use for your study. If you have previous versions of the consent templates saved, please delete them and download the new and improved templates for future use.
